To Support the New Parent Support Program for US Military Families Germany as a Contractor

  • Full Time Employment – 40 Hour Per Week

Qualifications Needed:

  • Bachelor’s degree in nursing-
  • Must possess a license as an RN in the US.
  • 2 years beyond degree of direct experience in child maltreatment, child abuse prevention, dysfunctional couples, families experiencing stress, violent hostile individual clients, parenting programs or related areas.

Job Requirements:

  • Providing infusion services in home
  • Assist with patient home visits
  • Ensuring in-home care delivery
  • Supervise home health contracting and authorizations
  • Communicating, and educating home care agency nurses
  • Solving and troubleshooting home pumps in the field
  • Provide proper home care for the patient referred to CHS/CHI NHC
  • Identifying patients who could benefit from home infusion services
  • Promote home health referrals in the health care community
  • Obtaining, maintaining, and executing home health agency contracts
  • Meet patient home infusion care needs and other care needs
  • Assisting physicians in treating patients in home and clinic setting
  • Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services
  • Assist with field orientation of newly hired home health staff members
  • Communicate any problems incurred in the delivery of home infusion therapies
  • Assess patient and family readiness and ability to perform dialysis treatments at home and patients' home environment for suitability in the ongoing delivery of home dialysis therapy
  • Oversee the delivery of home dialysis therapy to the patient in compliance with standards outlined in the FMS Home Peritoneal Dialysis Policy and Procedure Manual, or/Home Hemodialysis
  • Reconciling, education, and management of patient medications in the home
  • Attending physicians to obtain verbal or written orders for home care services
  • Approach members and customers and provide with the opportunity to schedule a no cost no obligation estimate in home for any of the home improvement products
